Terms & Conditions
Please read these terms carefully before you book our classes. These terms tell you who we are, how we will provide classes to you, how you and we may change or end the contract, what to do if there is a problem and other important information.
All those wishing to attend a Midland Futsal Academy session must agree to the terms and conditions set out below. The Terms shall be deemed to have been accepted by the Parent upon payment of the Fee. Payment by the Carer shall be deemed to have been made on behalf of the Parent.
1.Booking, Payment & Cancellation
1.1 You must complete a registration form in order to book a class with us.
1.2 Our class timetables are correct at the time of publishing. If by any chance your class/time changes, Midland Futsal Academy will be in touch with you. During the Easter and Summer breaks we tend to run on a reduced schedule, where we only have 1 evening class instead of 2. Other session bookings will be moved to the other class on the same day.
1.3 Your booking is not confirmed until payment has been received (you will receive an email to you when the booking has been confirmed). Paid spaces will be given priority.
1.4 Your subscription booking session payment has reserved a place at Midland Futsal Academy just for you and if you miss a session for any reason we are unable to refund you in any way nor another session. Single session bookings have to give 24+ hours notice, prior to the session, in order to claim a roll over session. We are unable to give out payment refunds.
1.5 If your child suffers an injury that leaves your child out for 2+ weeks and have sessions booked then please let us know as soon as possible we will then carry over your sessions for you when your child is able to attend back to Midland Futsal Academy. Unfortunately, we will not offer payment refunds.
1.6 If your session fee changes you will be notified by Midland Futsal Academy.
1.7 In the unlikely event of last minute session changes or a session cancellation by us, Midland Futsal Academy will notify you with as much notice as possible via email and we will post on our facebook page. Sessions that are cancelled due to a venue closure, your session will carry over to next month (subscription bookings) or next weeks session (single session bookings).
1.8 Book a Trial- We offer a 1 free trial to all new students. If your child has previously attended one of our sessions and has been booked on for a trial, you will be asked to pay the session fee.
1.9 Subscriptions- The total cost of your monthly subscription is calculated by the number of sessions in a year and divided into 12 equal payments. This payment is required and billed on the 1st of every month.
Subscription rates are calculated with the understanding that some sessions may be missed, as a result, no refunds will be issued for missed sessions due to child absence.
We reserve the right to adjust subscription rates at any time. In the event of a price change, parents will be notified in advance before their next payment.
You may cancel or pause your subscription at any time. Please make us aware 48 hours before the start of the month to avoid any costs
No pro-rata refund will be issued for the remaining sessions within the calendar month in which the cancellation occurs. Neither can these sessions be transferred
1.10 Camps- Cancellations made less than 7 days before the camp start date will be eligible for a credit of equal value to the original booking.
2.Parents Responsibility
The Parent/Carer warrants and represents that:
2.1 The information set out in the Registration Form is completed and accurate in all respects and the Parent/Carer will notify Midland Futsal Academy of any changes in such information immediately to info@midlandfutsalacademy.co.uk
2.2 When attending the session, neither the Pupil and Parent/Carer will be suffering with any contagious illness, disease, injury etc.
2.3 It is the Parent/Carer responsibility to inform Midland Futsal Academy of any condition that could affect your child’s capabilities during sessions. This can be updated by your child’s registration form. All information provided to us will be treated as strictly confidential.
2.4 The parent acknowledges and agrees that the pupil shall be deemed to be under the care and control of the Parent/Carer throughout the session and the Parent/Carer will be responsible for the welfare and conduct of the pupil throughout the session and whilst the Pupil is on the premises.
2.5 Customers are expected to behave in a civilised and respectful manner at all times towards the coaches, other customers and any members of public who are on the premises. In the event that Midland Futsal Academy determines the behaviour of the Pupil and/or the Parent/Carer to be unacceptable, Midland Futsal Academy have the right to exclude the Pupil and the Parent/Carer from Course and the premises permanently
2.6 Please ensure when attending the session you must be with your child at the gate/sports hall when dropping them off and picking them up.
